Employee Check-off Log (Blank Template)

Employee Check-offs are a way to track/log which employees have been trained on a new or updated job task.

Each task should have its own check-off log associated with it and be used in combination with the related Risk Assessment, Safe Work Practice, and Competency Checklist.

The Employee Check-off Log Blank Template can be used as part of any Task Management Package, and should be customized to fit applicable task information for your worksite.

Both versions capture the same information.

• Use V.2.1 if you have a large amount of employees to track.
• Use V.2.2 if you have a smaller amount of employees to track.
